Tennessee State Required Immunizations
By Tennessee State law, a child may not start in our program unless a TENNESSEE immunization form is received and on file before the start date. Families may have their child's pediatrician, family physician, or health department fax a copy to us at 423-265-0025 attention Bright School Nurse. They may also bring in a copy from the physician or email it to nurse@brightschool.com.
(SPECIAL NOTE: All kindergarten students require an updated immunization form before starting school, as mandated by the Tennessee Department of Health since the immunization requirements change at this time.)
We understand that relocating to a new state can be overwhelming. Rest assured, we're here to assist you with the transition. If you've moved here from another state, it's essential to have your child's most recent immunizations transferred to an official State of TN immunization form. If you have a local pediatrician, they can handle this for you. If not, the Hamilton County Health Department is ready and willing to assist. You can reach them at 423.209.8050 to initiate this process (which typically takes a week to complete).
Families with a medical exemption for state immunizations must provide documentation from their child's pediatrician or family physician stating the medical reasons for not being immunized. The state of Tennessee recognizes religious exemptions for immunizations. Please note that this form must be notarized if you submit an exemption for religious reasons.

Bonnie brings a strong background in nursing experience. She earned her BSN from Vanderbilt University and holds an undergraduate degree in biology from Indiana University. In addition, she is a Certified Diabetes Care and Education Specialist (CDCES).
